What should go in Personal Details and References?

Personal Details is for the practical facts some employers ask for — nationality, languages, driver's licence — one "Label: Value" per line. For References, list a name, role, company and contact per line, or leave the field blank and the resume prints "Available on request."

Should I include a photo?

That depends on country and industry norms. This template is photo-free by design, which is the safe default for most international applications.
