What makes a recommendation persuasive
Vague praise (“a great worker”) carries little weight; a concrete example with a measurable result does. Name the situation, what the person did, and the outcome. If you would happily rehire them, say so explicitly.
Being honest
Only agree to write a letter if you can be genuinely positive. If you cannot, it is kinder to decline than to write something faint — a tepid letter can hurt more than none.

Who should I address it to?
'To whom it may concern' is the safe default when you do not know the recipient. If you know the hiring manager or programme, address them by name.
How long should it be?
Three short paragraphs on one page is plenty. Recruiters skim; specifics in a short letter beat generalities in a long one.
